By signing below, I confirm that I am either the participant named on this form, or, if the participant is under 18, the participant’s parent or legal guardian.
I confirm that I have authority to sign this form and, if signing for a minor, give permission for the participant to participate in Building the Future workshops, classes, activities, and related events.
I understand that Building the Future activities may include educational instruction, hands-on learning experiences, demonstrations, workshops, mentoring, field activities, tools, equipment, materials, physical movement, dust, noise, and other supervised activities.
I acknowledge that participation may involve risks, including but not limited to cuts, scrapes, bruises, burns, eye injury, allergic reactions, illness, accidents, property damage, personal injury, and other unforeseen hazards, including injuries that may be serious. I voluntarily choose to participate, or allow the participant named on this form to participate, with knowledge of these risks.
For adult participants signing for themselves: I release, waive, and hold harmless Building the Future, its directors, officers, employees, volunteers, instructors, sponsors, property owners, affiliates, and representatives from claims, liabilities, damages, costs, or expenses arising out of my participation in Building the Future activities, including claims based on ordinary negligence, to the fullest extent permitted by law. This release does not apply to gross negligence or willful misconduct.
For parents or legal guardians signing for a minor participant: I acknowledge the risks described above, give permission for the minor participant to participate, and agree to be responsible for ensuring the participant understands and follows all safety rules, instructions, and staff directions.
In the event of an emergency, I authorize Building the Future and its representatives to seek emergency medical treatment for the participant if the emergency contact cannot be reached promptly. I understand that I am responsible for any resulting medical expenses.
I understand that Building the Future is committed to providing a safe, respectful, and educational environment. Participants are expected to conduct themselves appropriately, follow all safety rules, use required safety equipment, follow staff instructions, and treat others with respect. I understand that failure to follow safety rules or staff instructions may result in removal from the activity.
I understand that a completed waiver is required before participation, and that registration alone does not guarantee participation if this waiver has not been completed.
I have read and understand this agreement, and I voluntarily agree to its terms.
